Report a Safety Incident
Sawgrass Nutra Labs is committed to maintaining a safe workplace. Reporting safety incidents, injuries, hazards, and near-miss events helps prevent harm and supports continuous improvement.
Employees are encouraged to report safety concerns promptly and in good faith. There will be no retaliation for raising safety concerns.
Immediate Danger or Emergency?
If there is an immediate threat to life, health, or property:
- Call 911 immediately
- Move to a safe location if possible
- Notify site leadership as soon as it is safe to do so
Emergency response should always take priority over reporting.
What Should Be Reported
- Workplace injuries or illnesses
- Near-miss events (incidents that could have caused injury)
- Unsafe conditions or hazards
- Equipment failures or malfunctions
- Chemical spills or exposures
- Any situation that could impact employee safety or health
Reporting near-miss events is especially important, even if no injury occurred.
When to Report
Safety incidents should be reported as soon as possible after the situation is under control. Prompt reporting allows appropriate response, investigation, and corrective action.
How to Report a Safety Incident
- Notify your Supervisor or Manager
- Contact Environmental Health & Safety (EHS)
- Submit a Safety Incident Report using the approved reporting method

Confidentiality & Non-Retaliation
Reports will be handled as confidentially as possible. Retaliation against any individual who reports a safety concern or participates in an investigation is strictly prohibited.
After a Report Is Made
Reported incidents may be reviewed by Environmental Health & Safety, management, or other designated personnel to determine appropriate corrective or preventive actions.
